Drop a 5/8" bolt into the pitman arm alone, and see if there's play. It should be relatively snug. I would be inclined to think the hole in the pitman arm is elongated.

You can get it patched up. I had mine done recently. It cost $285 I believe (I'd have to double check the price). It took them half a day, and they've got a 3 year warranty. So far, so good.
